5 Warning Signs You Might Need a Root Canal (Don't Ignore These)

Root canals have an unfair reputation. In reality, they relieve pain — they don't cause it. Knowing the warning signs early can save your tooth and prevent a much more serious problem. Here's what to watch for.

A root canal sounds scary, but here's the truth: the procedure itself is no more uncomfortable than getting a filling. What's actually painful is the untreated infection that makes a root canal necessary. Catching the warning signs early means we can save your tooth before things get worse.

1. Severe, Persistent Toothache

A toothache that doesn't go away — especially one that's deep, throbbing, or radiates to your jaw — is one of the clearest signs that the nerve inside your tooth may be infected or dying. Over-the-counter pain relievers may temporarily dull the pain, but they won't treat the underlying problem.

2. Prolonged Sensitivity to Hot or Cold

Most people experience brief sensitivity to hot or cold foods. That's normal. But if the sensitivity lingers for more than 10–30 seconds after the stimulus is removed, it suggests the nerve is compromised and may need treatment.

3. Darkening of the Tooth

When a tooth turns gray, dark yellow, or brown — especially if it happened relatively quickly — this often signals that the inner pulp tissue is dying. The discoloration comes from the breakdown of the blood vessels inside the tooth.

4. Swollen, Tender Gums Near the Tooth

Swelling, tenderness, or a small pimple-like bump on the gum near a painful tooth (called a dental abscess or fistula) is a serious sign of infection. This means bacteria have spread beyond the tooth itself. This needs prompt treatment — dental infections can spread to the jaw, neck, and beyond if left untreated.

5. Pain When Chewing or Touching the Tooth

If biting down or even lightly touching a specific tooth causes a sharp pain, the ligaments around the tooth's root may be inflamed due to infection. This type of pain typically won't resolve without treatment.

What Happens If You Wait?

Dental infections don't resolve on their own. They can spread to surrounding teeth, the jawbone, and in severe cases, to other parts of the body. Waiting also increases the likelihood that the tooth can't be saved and will need extraction instead — a more expensive and involved process.

A root canal involves removing the infected pulp from inside the tooth, cleaning and shaping the root canals, and sealing everything with a crown. With modern anesthesia, most patients report feeling nothing more than pressure. The relief afterward is almost immediate.
